Policies and Forms

Having the self-awareness and courage to seek counseling for issues you have not successfully addressed on your own is the first part of your journey to a more positive life.

Insurance

The coverage you have for mental health services depends on your provider’s or employer’s plan. It can range from full to limited or no coverage. Check with your employer’s benefits office or your insurance provider to determine coverage for out-of-network benefits.

Questions you may want to ask:

  • Are mental health services covered in my plan?
  • What is my deductible, and have I met all or part of it?
  • What are the limits in my plan related to sessions per year and other issues?
  • How do I make an insurance claim for out-of-network services?

Willow Oaks accepts these employee assistance programs (EAPs):

  • Access
  • Carebridge
  • Mazzitti & Sullivan
  • Quest
  • WellSpan

Payment

Willow Oaks accepts cash, check, Health Savings Accounts, and all major credit cards.

Cancellation Policy

Sometimes unexpected circumstances arise that prevent you from being able to attend a session. If that happens to you, I ask for 24 hours or more notice to avoid being charged for your session.

Other Questions

I welcome any other questions related to sessions, therapy, policies or other issues. Answering your questions can lead to greater counseling success.

Forms

I suggest that new clients consider filling out Willow Oaks’ intake form prior to our first session to save time and get right to work. However, you can complete that form at our first session if you prefer.
